Prescription Refill

We are happy to help you refill you prescription. Please fill out the form below to submit your request. Your refill may take a few days to fill based on the amount of requests and type of medication. 

All prescription refill requests are to be submitted 7-10 business days prior to the due date.

The OrthoIndy staff will contact the patient via telephone once the requested prescription(s) has been approved and either called to the pharmacy, ready for pick-up, or prepared to mail.

Please DO NOT make multiple requests for the same medication, as this will only delay processing time.

Per DEA regulation, Schedule II medications cannot be called to the pharmacy; therefore, the written prescription must be mailed or picked-up at an OrthoIndy location. If requesting a refill on a Schedule II medication, please select one of the options above.
If you selected "Mail" for Schedule II Medications, please tell us your address so we can mail your prescription.
